2. Print the Form at Your end.
3. Fill-in the form and send it along with Rs.100 Demand Draft/Pay Order/Local Check Payable at Ahmedabad in favor of "Diwan-Ballubhai Alumni Association" (For Outstation Check Add Rs.100 i.e. send Rs.200 Check).
4. Please do not forget to write Payment Details on the Form.
5. Attached copy of School Leaving Certificate and/or Other School Documents for Proof
6. Send/Post it to: Diwan-Ballubhai Alumni Association, Diwan-Ballubhai High School, Kankaria, Ahmedabad-380022.
